Gaps in community-level early detection of non-communicable diseases (NCDs) and injury-management literacy call for simple, integrated interventions. This article reports a community service model that integrates basic health screening (blood pressure, anthropometrics), five-phase injury rehabilitation education (pain control–mobility–strength–neuromuscular coordination–sport-specific), and kinesiology taping (KT) demonstrations as a safe adjunct. The program was co-designed with local partners, aligned with measurement guidelines and return-to-sport principles. Implementation comprised registration and consent, a screening station with brief counseling and referral, an interactive mini-lecture with KT practice, and dissemination of reusable modules/infographics and standard operating procedures (SOPs). Descriptively, the program achieved strong community engagement, identified participants needing clinical follow-up, improved understanding of staged recovery, and yielded positive acceptance of KT as a complement to exercise. Conclusion: This model is practical, low-cost, and readily replicable to strengthen health literacy, link at-risk participants to primary care, and embed an exercise-based rehabilitation paradigm. Recommendations include integration with Posbindu/primary health center (PKM) networks, cadre training, and periodic follow-up sessions.
