Abstract

Abstrak

Perencanaan radioterapi merupakan tahapan penting dalam penatalaksanaan kanker yang bertujuan memastikan distribusi dosis optimal pada target tumor serta meminimalkan paparan terhadap Organ at Risk (OAR). Pada kasus kanker nasofaring, pemilihan teknik penyinaran yang tepat diperlukan karena lokasi tumor berdekatan dengan organ vital. Penelitian ini untuk mengevaluasi perencanaan radioterapi menggunakan teknik Intensity Modulated Radiation Therapy (IMRT) dan Volumetric Modulated Arc Therapy (VMAT) pada pasien kanker nasofaring menggunakan pendekatan deskriptif-komparatif terhadap sepuluh data pasien kanker nasofaring dengan TPS Monaco versi 6.1.1.0 dan metode inverse planning. Evaluasi target menggunakan V98, V95, dan V107, sedangkan OAR menggunakan Dmax dan Dmean. Karena pasien yang sama dibandingkan dengan dua teknik, analisis inferensial dilakukan pada data berpasangan menggunakan uji Shapiro–Wilk, paired t-test, atau Wilcoxon signed-rank test sesuai distribusi selisih pasangan. Hasil Penelitian menunjukkan kedua teknik memberikan cakupan dosis yang optimal pada target volume. Nilai V98 dan V95 pada GTV dan CTV sebagian besar mendekati 100%, sedangkan nilai pada PTV berada dalam batas toleransi klinis yang diterima. Nilai V107 pada PTV70 berada di bawah 10%, sehingga menunjukkan hot spot yang aman secara klinis. Evaluasi terhadap OAR menunjukkan dosis pada brainstem, spinal cord, serta parotid kanan dan kiri berada dalam batas toleransi klinis. Perbandingan menunjukkan perbedaan dosimetri antara IMRT dan VMAT kecil, namun VMAT memberikan dosis lebih rendah pada parotid untuk beberapa kasus. Kesimpulannya, teknik IMRT dan VMAT menghasilkan cakupan dosis target yang serupa. VMAT memberikan Dmean parotid kanan yang lebih rendah secara bermakna, sedangkan perbedaan PTV70, brainstem, spinal cord, dan parotid kiri tidak bermakna secara statistik. Dosis parotid kiri pada pasien tertentu dapat melebihi target klinis sehingga evaluasi OAR perlu mempertimbangkan karakteristik individual pasien.

Abstract

Radiotherapy planning is an important stage in cancer management that aims to ensure optimal dose distribution to the tumor target while minimizing exposure to Organs at Risk (OAR). In nasopharyngeal cancer cases, selecting an appropriate irradiation technique is necessary because the tumor location is close to vital organs. This study aims to evaluate radiotherapy planning using Intensity Modulated Radiation Therapy (IMRT) and Volumetric Modulated Arc Therapy (VMAT) techniques in nasopharyngeal cancer patients used a descriptive-comparative approach on ten nasopharyngeal cancer patient datasets using the Monaco TPS version 6.1.1.0 and the inverse planning method. Target evaluation used V98, V95, and V107, while OAR evaluation used Dmax and Dmean. Since the same patients were compared using both techniques, inferential analysis was performed on paired data using the Shapiro–Wilk test, paired t-test, or Wilcoxon signed-rank test according to the distribution of the paired differences. The results show that both techniques provide optimal dose coverage to the target volume. The V98 and V95 values for GTV and CTV are mostly close to 100%, while the values for PTV fall within an acceptable clinical tolerance range. The V107 value for PTV70 is below 10%, indicating that the resulting hot spots are clinically safe. Evaluation of the OAR shows that doses to the brainstem, spinal cord, and both right and left parotid glands fall within clinical tolerance limits. The comparison shows that dosimetric differences between IMRT and VMAT are small, though VMAT tends to provide lower doses to the parotid in some cases. In conclusion, the IMRT and VMAT techniques produce relatively similar target dose coverage. VMAT provides a significantly lower right parotid Dmean, while differences in PTV70, brainstem, spinal cord, and left parotid are not statistically significant. Left parotid dose in certain patients may exceed the clinical target, so OAR evaluation still needs to take individual patient characteristics into account.
