Gambaran Psikologis Pasien dengan Depresi Berat Tanpa Gejala Psikotik Di RSJ X

Gangguan depresi berat merupakan masalah kesehatan mental yang berdampak signifikan terhadap fungsi emosional, kognitif, sosial, dan fisik individu. Depresi berkembang melalui interaksi antara kerentanan psikologis dan stresor kehidupan yang berkepanjangan. Studi kasus ini bertujuan untuk menggambarkan dinamika psikologis dan faktor-faktor yang berperan dalam perkembangan depresi berat tanpa gejala psikotik pada dewasa awal. Penelitian menggunakan desain studi kasus klinis dengan pendekatan kualitatif deskriptif. Subjek adalah perempuan dewasa awal berusia 27 tahun yang menjalani asesmen psikologis di RSJI X. Data dikumpulkan melalui observasi klinis, autoanamnesis, alloanamnesis, serta studi dokumentasi rekam medis. Hasil asesmen menunjukkan subjek memenuhi kriteria F32.2 Depresi Berat Tanpa Gejala Psikotik, ditandai gejala emosional, kognitif, perilaku, sosial, dan fisik yang signifikan, termasuk ide bunuh diri. Faktor dominan meliputi tekanan peran keluarga, kehilangan ibu, kegagalan hubungan, keterbatasan dukungan sosial, dan distorsi kognitif negatif. Depresi berat pada subjek dipengaruhi oleh interaksi kompleks antara faktor psikologis dan lingkungan, sehingga memerlukan intervensi komprehensif berbasis kognitif serta dukungan keluarga.
Abstract
Severe depressive disorder is a mental health condition that significantly impairs emotional, cognitive, social, and physical functioning. It develops through the interaction between psychological vulnerability and prolonged life stressors. This case study aims to describe the psychological dynamics and contributing factors of severe depressive disorder without psychotic features in early adulthood. A qualitative descriptive clinical case study design was applied. The subject was a 27-year-old early adult female undergoing psychological assessment at RSJ X. Data were collected through clinical observation, auto-anamnesis, allo-anamnesis, and medical record review. The assessment revealed that the subject met the criteria for F32.2 Severe Depressive Disorder without Psychotic Features, characterized by significant emotional, cognitive, behavioral, social, and physical symptoms, including suicidal ideation. Contributing factors included family role pressure, maternal loss, relationship failure, limited social support, and negative cognitive distortions. Severe depression in this case is shaped by the interaction of psychological vulnerability and environmental stressors, indicating the need for comprehensive cognitive-based interventions and family support.
